As part of our Health & Wellness Days, we have Heart Health clinics where patient blood samples are screened on site for total cholesterol and blood glucose levels by our pharmacy team. Blood pressure is taken with automatic monitors. Your results for these tests are available immediately.

This is convenient for patients who may not be routinely following-up with a doctor or are monitoring in-between visits. 

 

PROfile Medication reviews are performed by our Pharmacists on our patient files. These assessments are carried out depending on the medication or conditions being treated. If appropriate, they may be performed weekly if doses are changing often.

Reviews may be initiated by anyone: the patient, the patient's family, caregiver, the doctor, or if the pharmacist sees a need themselves. Other PROfile programs, such as Pill Pack Plus and AutoFill Plus create opportunities for more frequent reviews.

 

When a new prescription is picked up, your PROfile Pharmacist will counsel you on the proper way to take it, how the medication works, expectations, side effects, etc. Follow-up calls are performed by your PROfile Pharmacist in order to reinforce these points, monitor the effectiveness of the therapy, and answer or address any further questions or concerns. The calls are made in a time period depending on the therapy. Calls may coincide with a doctor’s appointment or a blood test, if that information is available.

 

Quality patient care is built on relationships. These are between patient-pharmacist, patient-physician, pharmacist-physician, pharmacist-RN, etc. Communication is key to making these relationships work, and your PROfile Pharmacist is proactive in addressing issues efficiently and professionally.
